My Revive Inc is committed to safeguarding your privacy. This Privacy Policy (this “Policy”) describes how MY REVIVE may collect, use, share, and keep information about you and what choices you have. This Policy applies to MY REVIVE’s websites, apps, software platforms, or online offerings (collectively, the “Platform”), and will be available by link on all sites and offerings which it covers. You should review the applicable privacy policy available. By using or accessing the Platform in any manner, you acknowledge that you accept the practices and policies outlined in this Policy, and you hereby consent that we will collect, use, and share your information in accordance with this Policy.

Your continued use of MY REVIVE’s Platform is at all times subject to the Terms of Use, which incorporate this Policy. Any terms not defined in this Policy shall have the same definitions given to them in the Terms of Use.

MY REVIVE reevaluates this Policy on an ongoing basis and reserves the right to change its Policy at any time in its sole discretion. All changes to the terms of this Policy shall be effective as of the date on which they are posted on the Platform. It is your responsibility to check this page periodically for any such changes or updates. Your continued use of or access to the Platform or our services following the posting of any changes, modifications, or updates constitutes your express acceptance thereof.

I. PRIVACY POLICY COVERAGE

This Policy covers our treatment of personally identifiable information (“Personal Information”) that MY REVIVE gathers when you are accessing or using our Platform. We gather various types of Personal Information from our users, as explained in more detail below, and we use this Personal Information internally in connection with our Platform, including to enhance and personalize your experience with MY REVIVE, to contact you and allow other users to contact you, to fulfill your requests for certain products and services, and to analyze how you use the Platform. MY REVIVE may also share some Personal Information with third parties, but only as described below.

MY REVIVE’s Platform and offerings are directed to individuals over the age of eighteen (18). They are not intended for children under eighteen (18) years of age. MY REVIVE does not knowingly solicit data online from, or market online to, children under the age of thirteen (18) years. In addition, children under the age of eighteen (18) years are not permitted to submit any personal information to MY REVIVE or the Platform. If we learn that we have collected personal information from a child under age 18, we will delete that information as quickly as possible. II. INFORMATION MY REVIVE MAY COLLECT

MY REVIVE may gather date, time, information about a user’s browser and system or device configuration and capabilities, navigation history, and IP address for all visitors to the Platform to monitor and track Platform use and traffic patterns in order to provide a more personalized experience on the Platform. MY REVIVE may use this information for its internal security audit log, for trend analysis and system administration, and to gather broad information about its audiences. However, MY REVIVE  does not routinely link IP addresses to anything that identifies you as an individual.

MY REVIVE may also use cookies and other commonly used data collection technologies on the Platform and in its newsletters and e-mails to collect information on both an aggregate and individual basis.

In addition to the above, we may use the following technologies to automatically collect information about your activities on our Platform: cookies, flash cookies, web beacons, clear pixels, or pixel tags, web server logs, and geo-location technologies.

Having accurate information about you permits us to provide you with a smooth, efficient, and customized experience. Specifically, we may use information collected about you via the Platform to:

  1. Administer sweepstakes, promotions, and contests;
  2. Assist law enforcement and respond to subpoena;
  3. Compile anonymous statistical data and analysis for use internally or with third parties;
  4. Create and manage your account;
  5. To help you connect with other users;
  6. Enable features that allow you to share information with other users, such as when you submit a review or rating;
  7. Email you regarding your account;
  8. Enable user-to-user communications;
  9. Generate a personal profile about you to make future visits to the Platform more personalized;
  10. Increase the efficiency and operation of the Platform;
  11. Monitor and analyze usage and trends to improve your experience with the Platform;
  12. Notify you of updates to the Platform;
  13. Offer new products, services, mobile applications, and/or recommendations to you;
  14. Perform other business activities as needed;
  15. Prevent fraudulent transactions, monitor against theft, and protect against criminal activity;
  16. Process payments and refunds;
  17. Request feedback and contact you about your use of the Platform;
  18. Resolve disputes and troubleshoot problems;
  19. Respond to product and customer service requests;
  20. Send you a newsletter;
  21. Solicit support for the Platform;

“Personal Information” shall refer to information which you provide to us which personally identifies you, and includes, but is not limited to, your name, gender, email address, social security number, mailing and/or physical address, mobile device number, telephone number, date of birth, your photograph and other information or data that identifies you, which is collected and maintained by us if you have an Account to use and access the Platform, and may otherwise be collected by us through your use and access of the Platform or if you otherwise directly provide such information to us. “Personal Information” may also include information you provide in connection with a payment or banking service (“Payment Service”) offered as a convenience to you in association with the Platform services, whereby you can maintain your credit card or bank account information (each a “Payment Account”) securely with us in connection with your Account. Such information may also include, depending on whether it can be reasonably linked.

to your other Personal Information or used to identify you, your IP address, MAC address, device specific information (including hardware information about any device you are using to access or use the Platform, such as your UDID or hardware model information), device type, unique device identifiers, advertising identifiers, serial numbers, mobile network information, file names and versions, preferred languages, operating system information (including your operating system version), browser type, location information, user preferences, dates and times of use of the Platform, browsing activity and browsing history, and features you have used (collectively, “Usage Data”). Personal Information may also include certain combinations of such information that meets the applicable statutory or other legal definition of “Personal Information” for purposes of breach notification (such information, “Statutory PII”).

MY REVIVE may collect this information through a website or a mobile application. By using the Services, you are authorizing us to gather, parse, and retain related to the provision of the Services.

  1. Information you Provide. To register as a user with MY REVIVE, you will be asked to provide certain information, including your Personal Information. You will also be asked to allow MY REVIVE  to collect your location information from your device when you download the App or use the Services. In addition, MY REVIVE may collect and store any Personal Information you provide while using the Services or in some other manner. This may include identifying information, such as your name, address, email address, and telephone number, and, if you transact business with MY REVIVE, financial information, government identification numbers, and birth date. If you contact MY REVIVE with a customer service or other inquiry, you provide MY REVIVE with the content of that communication. If you provide us your phone number, you may also receive messages that we send you via SMS as part of the normal business operation of your use of the Platform.   2. Use of Technologies to Collect Information. MY REVIVE may use various technologies to collect information from your device about your activities in connection with its Services.

(i) Information Collected Automatically.

Whenever you interact with our Platform, we automatically receive and record information on our server logs from your browser or device, which may include your IP address, geolocation data, device identification, “cookie” information, web beacons, the type of browser and/or device you’re using to access our Platform, and the page or feature you requested. “Cookies” are identifiers we transfer to your browser or device that allow us to recognize your browser or device and tell us how and when pages and features in our Platform are visited and by how many people. You may be able to change the preferences on your browser or device to prevent or limit your device’s acceptance of cookies, but this may prevent you from taking advantage of some of our features. A “web beacon” is an object that is embedded in a page or email and is usually invisible to the user but allows checking whether a user has viewed the page or email. We may use this data to customize content for you that we think you might like, based on your usage patterns.

If you click on a link to a third-party website or service, a third party may also transmit cookies to you. This Policy does not cover the use of cookies by any third parties, and MY REVIVE  shall not be responsible for third parties’ privacy policies and practices. MY REVIVE does not access or control these third-party cookies or web beacons; however, we may have access to data generated from their use, such as when we view analytics reports provided by such third-party tools and services. Please note that cookies placed by third parties may continue to track your activities online even after you have left our Platform, and those third parties may not honor the “Do Not Track” requests you have set using your browser or device.

(ii) Collection of Location Information

When you use the Platform, we may collect precise location data that tracks your location when the mobile application is running in the foreground (mobile application open and on-screen) of your device. We may deliver information or content specific to your location to you via the Platform as a result of the location data we collect. Your location information may be necessary to be able to personalize the app experience, track the progress and completion of deliveries, assist in finding available markets or available delivery opportunities, and other similar purposes. If you disable the collection of geolocation information, the Platform may not function as intended. Additionally, the opt-out mechanism on some operating systems may not be persistent, which will require you to opt-out of such collection each time you access the Platform through your device.

(iii) Transaction Information

We collect transaction details and information related to your use of the Platform, including the types of services you requested or provided, dates, times, and other related transaction details and information.

(iv) Device Information

We may collect information about the devices you use to access our Platform, including IP address, web browser type, mobile operating system version, phone carrier and manufacturer, unique device identifiers such as your device type, and any related Usage Data.

(v) Log Information

When you interact with our Platform, we collect server logs, which may include information about your interactions with our Platform, such as Usage Data, device IP address, access dates and times, application features or pages viewed, application crashes and other system activity, browser type, and the third-party site or service you were using before interacting with our Platform, to help us improve our Platform, and help us understand how you use our Platform.

(vi) Information Collected from Third-Party Websites and Do Not Track Policy

Through cookies we place on your browser or device, we may collect information about your online activity after you leave our Platform. Just like any other usage information we collect, this information allows us to improve the Platform and customize your online experience, and otherwise as described in this Policy. Your browser may offer you a “Do Not Track” option, which allows you to signal to operators of websites and web applications and services (including behavioral advertising services) that you do not wish such operators to track certain of your online activities over time and across different websites. Our Platform does not support Do Not Track requests at this time, which means that we collect information about your online activity both while you are using the Platform and after you leave our Platform.

(vii) Calls and Text Messages

MY REVIVE may use this information for customer support services (including to resolve disputes), for safety and security purposes, and for analytics. Additionally, in order to provide the services to you, MY REVIVE will communicate with you with communications or through its alert service, which includes contacting or messaging you by telephone, email, short message service or text message. Additionally, MY REVIVE and its partners and affiliates may contact you at any telephone number(s) you provide in connection with your use of the Platform. By providing your contact information, you consent to receive communications (including communications containing an artificial voice or prerecorded message or made using an automatic telephone dialing system) from us, our affiliates and partners, and any other user from which you received a match at any of the contact information you provided, including your wireless number, even if that number is registered on a corporate, state, or national do-not-call registry.

(viii) Cookies and Use of Cookie Data.

When you visit the Platform, MY REVIVE may assign your device one or more cookies to facilitate access to the Platform and to personalize your experience. Through the use of a cookie, MY REVIVE also may automatically collect information about your activity on the Platform, such as the pages you visit, the time and date of your visits, and the links you click. If MY REVIVE advertises, MY REVIVE (or third parties) may use certain data collected on the Platform to show you MY REVIVE ‘s advertisements on other sites or applications.

(iv) Information Collected by Third Parties.

MY REVIVE may allow service providers, advertising companies and ad networks, and other third parties to display advertisements on MY REVIVE‘s Platform. These companies may use tracking technologies, such as cookies or web beacons, to collect information about users who view or interact with their advertisements. MY REVIVE does not provide any non-anonymized personal information to third parties. Some of these third-party advertising companies may be advertising networks that are members of the Network Advertising Initiative, which offers a single location to opt out of ad targeting from member companies (www.networkadvertising.org). Opting out will not decrease the number of advertisements you see.

III. HOW MY REVIVE USES COLLECTED INFORMATION

  1. In General.  MY REVIVE may use information MY REVIVE collects about you to:
    • Deliver and improve MY REVIVEs products and services, and manage MY REVIVE‘s business.
    • Enable transportation, deliveries and other services.
    • Process or facilitate payments for those services.
    • Enable features that allow you to share information with other people, such as when you submit a comment about a driver, or refer a friend.
    • Manage your account and provide you with customer support.
    • Perform research and analysis about your use of, or interest in, MY REVIVE’s or others’ products, services, or content.
    • Communicate with you by email, postal mail, telephone, and/or mobile devices about products or services that may be of interest to you either from MY REVIVE or other third parties; MY REVIVE Privacy Policy Page 6 of 9.
    • Develop, display, and track content and advertising tailored to your interests on MY REVIVE Platform and other sites, including providing MY REVIVE‘s advertisements to you when you visit other sites.
    • Website or mobile application analytics.
    • Verify your eligibility and deliver prizes in connection with contests and sweepstakes.
    • Enforce or exercise any rights in MY REVIVE’s Terms of Use; and
    • Perform functions or services as otherwise described to you at the time of collection.
  2. Job Applicants. If your personal information is submitted through MY REVIVE‘s website when applying for a position with MY REVIVE, the information will be used solely in connection with considering and acting upon your application. MY REVIVE may retain your personal information, but only for the purpose of considering your application for current or future available positions. This information, which may include your name and contact information, prior education and experience, and other information you provide to MY REVIVE, may be shared with third-party service providers retained by us to collect, maintain and analyze candidate submissions for job postings.
  3. Financial Information. MY REVIVE may use financial information or payment method to process payment for any purchases made on MY REVIVE‘s website, enroll you in the discount, rebate, and other programs in which you elect to participate, to protect against or identify possible fraudulent transactions, and otherwise as needed to manage MY REVIVE‘s business.

IV. WITH WHOM MY REVIVE MAY SHARE YOUR INFORMATION

  1. Information Shared with Other Users.
    When you register as a user of MY REVIVE, your MY REVIVE profile will be viewable by other users of the Platform. Other users will be able to view information you have provided to MY REVIVE directly, such as your photos and any additional photos you upload, your first name, your age, approximate number of miles away, your personal description, and information you have in common with the person viewing your profile. If you chat with another user, of course the other user will see the content of your chat.
  2. Personal Information.
    MY REVIVE does not share your personal information with others except as indicated in this Privacy Policy or when MY REVIVE informs you and gives you an opportunity to opt out of having your personal information shared. MY REVIVE may share personal information with: 

     

    • Service Providers: MY REVIVE may share information, including personal information, with third parties that perform certain services on MY REVIVEs behalf. These services may include fulfilling orders, providing customer service and marketing assistance, performing business and sales analysis, ad tracking and analytics, member screenings, supporting our website functionality, and supporting contests, sweepstakes, surveys and other features offered through MY REVIVE‘s site. MY REVIVE may also share your name, contact information and credit card information with MY REVIVE‘s service providers who process credit card payments. These service providers may have access to personal information needed to perform their functions but are not permitted to share or use such information for any other purposes.     • Other Situations. MY REVIVE may disclose your information, including personal information:

    • When MY REVIVE believes disclosure is appropriate in connection with efforts to investigate, or event, or take other action regarding illegal activity, suspected fraud or other wrongdoing, to protect and defend the rights, property, or safety of MY REVIVE, MY REVIVE‘s users, MY REVIVE‘s employees, or others, to comply with applicable law or cooperate with law enforcement, or to enforce BEST MY REVIVE‘s Terms of Use or other agreements or policies.
    • In connection with a substantial corporate transaction, such as the sale of MY REVIVE‘s business, a divestiture, merger, consolidation, or asset sale, or in the unlikely event of bankruptcy.
  3. Aggregate and/or Non-Personal Information.
    MY REVIVE may use and share non-personal information MY REVIVE collects under any of the above circumstances. MY REVIVE may also share it with third parties to develop and deliver targeted advertising on the Services and on websites or applications of third parties, and to analyze and report on advertising you see. MY REVIVE may combine non-personal information MY REVIVE collects with additional non-personal information collected from other sources. MY REVIVE also may share aggregated, non-personal information, or personal information in hashed, non-human readable form, with third parties, including advisors, advertisers, and investors, for the purpose of conducting general business analysis or other business purposes. For example, MY REVIVE may engage a data provider who may collect web log data from you (including IP address and information about your browser or operating system), or place or recognize a unique cookie on your browser to enable you to receive customized ads or content. The cookies may reflect deidentified demographic or other data linked to data you voluntarily have submitted to MY REVIVE (such as your email address), that MY REVIVE may share with a data provider solely in hashed, non-human readable form. V. HOW YOU CAN ACCESS YOUR INFORMATION

If you have a MY REVIVE account, you have the ability to review and update your personal information within the Platform by opening your account and going to settings. More information about how to contact MY REVIVE is provided below. You also may close your account at any time by visiting the “Settings” page for your account. If you close your account, MY REVIVE will retain certain information associated with your account for analytical purposes and recordkeeping integrity, as well as to prevent fraud, enforce MY REVIVE‘s Terms of Use, take actions MY REVIVE deems necessary to protect the integrity of the Platform or MY REVIVE‘s users, or take other actions otherwise permitted by law. In addition, if certain information has already been provided to third parties as described in this Privacy Policy, retention of that information will be subject to those third parties’ policies.

VI. YOUR CHOICES ABOUT COLLECTION AND USE OF INFORMATION

  • You can choose not to provide MY REVIVE with certain information, but that may result in you being unable to use certain features of the Services because such information may be required in order for you to register as user, purchase products or services, participate in a contest, promotion, survey, or sweepstakes, ask a question, or initiate other transactions.
  • The Platform may also deliver notifications to your phone or mobile device. You can disable these notifications by deleting the relevant Services or by changing the settings on your mobile device.
  • You can also control information collected by cookies. You can delete or decline cookies by changing your browser settings. Click “help” in the toolbar of most browsers for instructions.

VII. HOW MY REVIVE PROTECTS YOUR PERSONAL INFORMATION

MY REVIVE takes security measures to help safeguard your personal information from unauthorized access and disclosure. However, no system can be completely secure. Therefore, although MY REVIVE takes steps to secure your information, MY REVIVE does not promise, and you should not expect, that your personal information, chats, or other communications will always remain secure. Users should also take care with how they handle and disclose their personal information and should avoid sending personal information through insecure email. VIII. INFORMATION YOU PROVIDE ABOUT YOURSELF WHILE USING THE PLATFORM

MY REVIVE provides areas on the Services where you can post information about yourself and others and communicate with others. Such postings are governed by MY REVIVE‘s Terms of Use. Also, whenever you voluntarily disclose personal information on publicly-viewable pages, that information will be publicly available and can be collected and used by others. For example, if you post your email address, you may receive unsolicited messages. MY REVIVE cannot control who reads your posting or what other users may do with the information you voluntarily post, so MY REVIVE encourages you to exercise discretion and caution with respect to your personal information.

IX. VISITING THE SERVICES FROM OUTSIDE THE UNITED STATES

If you are visiting the Services from outside the United States, please be aware that your information may be transferred to, stored, and processed in the United States and globally where MY REVIVE‘s servers are located and MY REVIVE‘s central database is operated. By using the Platform, you understand and agree that your information may be transferred to MY REVIVE‘s facilities and those third parties with whom MY REVIVE shares it as described in this Privacy Policy.

X. NO RIGHTS OF THIRD PARTIES

This Privacy Policy does not create rights enforceable by third parties or require disclosure of any personal information relating to users of the Platform or App.

XI. CHANGES TO THIS PRIVACY POLICY MY REVIVE

will occasionally update this Privacy Policy. When MY REVIVE posts changes to this Privacy Policy, MY REVIVE will revise the “last updated” date at the top of this Privacy Policy. MY REVIVE recommends that you check the Platform from time to time to inform yourself of any changes in this Privacy Policy or any of our other policies.
